Licensed Practical Nurse (LPN) – Job Description
Position Summary
The Licensed Practical Nurse (LPN) is responsible for providing quality nursing care to residents by administering medications, monitoring resident health conditions, communicating changes to physicians and nursing leadership, supporting Certified Nursing Assistants (CNAs), and ensuring compliance with all healthcare regulations and facility policies.
Essential Job Duties
- Administer medications to residents in accordance with the “Five Rights” of medication administration.
- Monitor residents for changes in condition and promptly report concerns to the Charge Nurse and physician as appropriate.
- Communicate resident condition updates to family members, physicians, and other healthcare professionals when necessary.
- Assist CNAs with resident care tasks, including responding to call lights and supporting daily care activities.
- Observe and report any allegations or suspicions of abuse, neglect, or misappropriation immediately to the Director of Nursing and Administrator.
- Maintain accurate and timely documentation of nursing care and resident status.
- Ensure compliance with facility policies, state regulations, and nursing standards.
- Perform additional duties and responsibilities as assigned.
Required Qualifications
- Must possess and maintain a current Wisconsin Licensed Practical Nurse (LPN) license.
- Current CPR/BLS certification required.
- Minimum of one (1) year of related nursing experience preferred.
- Strong attention to detail and accuracy in medication administration and documentation.
- Excellent communication and interpersonal skills.
- Ability to work effectively as part of a multidisciplinary healthcare team.
